Transition Planning

When students turn 14 years old they enter into a formal process called Transition Planning.

Students, parents, and educational teams work together to map out specific goals for life after high school. Moving forward, these activities become a part of each student’s individualized education plan.

Although each plan is unique, some common goal areas include:

  • Self-Employment through a Micro-Enterprise
  • College Credit Plus
  • Post-secondary education
  • Community Participation
  • Vocational Education
  • Adult Services
  • Independent Living
  • Day Habilitation Programs

At our school the selection of transition goals is guided by students’ completion of the Assessment of Functional Living Skills, a series of assessments and comprehensive task analyses that extend across three domains:

  • Basic Living Skills
  • Home Living Skills
  • Community Participation Skills

One tool we use to unlock student progress towards transition goals is called IDAPT.

IDAPT is our very own, custom-built life and job skills training software. It offers a robust suite of on-the-job task supports. Photos, audio cues, videos, and text instructions are displayed on a tablet so each learners can gain greater independence.
