The Autism Model Community School accepts enrollment applications for new K-12 students based on completion of the student application form.

What You’ll Need

Some of the items you’ll be asked to provide in the student application include…

  • Individualized Education Plan (IEP or 504)
  • Evaluation Team Report (ETR)
  • Any other report(s) regarding student ability or need(s)
  • Proof of Ohio residency, such as a utility bill at the current address
  • Birth certificate showing child between ages of 5-21

How to Submit

You can send all attachments related to the student application by email (Admissions@amstoledo.org) or fax (419-897-4403).

What Happens Next?

After 7-10 business days, we’ll give you a call to confirm that we have received the student’s application and all needed attachments.

At this point, we’ll follow our waiting list procedures.

Down the road a bit, upon a possible student placement, we’ll contact you to schedule a campus tour.
